Rank Change	$50.00
Regular Certificate (Initial or Renewal)	*$50.00
Adding Area(s) of Certification	$50.00
Five-Year Substitute Certificate	$15.00
Duplicate Certificate (official copy)	$25.00
Extension of One-Year Certificate after KTIP year	**$35.00
* Renewal includes all previously approved certifications and endorsements
**Applicants who have recently completed the Kentucky Teacher Internship Program (KTIP) and are applying for their first Professional Certificate shall submit $35

Please mail application and certified/cashier's check/money order made payable to "Kentucky State Treasurer" to:
